Title | Analysis of the Primary Health Care Reform in Croatia: the Relevance of Policy Design |
Summary | The thesis analyzes health care reforms in Croatia, with a special focus on primary health care reforms. Thesis aims to answer a question: Why are the overall expenditures in health increasing, and quality of the services decreasing, even though the planned reforms envisioned cost reduction and higher quality of provided health services? Question is examined through the policy design lens and to answer the research question, theory of historical institutionalism and incremental institutional change is applied. Thesis uses qualitative methods, namely the analysis of the secondary data and semi-structured interviews with relevant stakeholders of Croatian health policy conducted by the author. The findings show that policy design influences policy outcomes. Reforms were designed in a path dependent way, thus achieving impact that was different from what was expected. Two main challenges in policy making can be observed. Firstly, continuous decline in PHC funding means that expenses of secondary and tertiary levels of care continue to increase. Secondly, reforms often create new policies without eliminating previous policies. As a result, incoherent health policy making can be observed. |
